Provide merged 4.1/4.2 upgrades

Description
Now that upgrading individual permissions is no longer necessary (#2874), 4.4 upgrade scripts should be provided from the last several versions. i.e.
- sql/psql/OMERO4.40/OMERO4.10.sql
- sql/psql/OMERO4.40/OMERO4.20.sql
as well as the new upgrade script:
- sql/psql/OMERO4.40/OMERO4.30.sql
These will largely be a concatenation of the individual scripts with the permission upgrade sections removed.

For some users still on older versions, skipping the painful "change every permission in the system" upgrade now that most are ignored would be VERY useful.
