Description
Viewing a plate with multiple fields-- fields not visible in web, but visible in insight.

Tested on Gretky using user-5
- The web now displays the field drop down menu.

Looks like this is related to a commit ([9dd2a21798091111c3a46591bde741f12a7d1ed0/ome.git]) that was made to address #9268. The form_well_index is basically not representative of the actual number of fields in the Plate.
