It seems on Insight on Windows, downloading the FileAnnotation? that is returned from a script fails if you do this from the script Activities dialog, but not if you download the attachment from the Image itself.

You may also have a look at Agilo extensions to the ticket.
I've tried reproducing this on a Windows 7 VM with both 5.0.1 and 5.0.2. No success. I'll have a look at the log files and see if connecting to a server running on Windows makes a difference.
