Bug: forgottenpassword URL is broken

After a request on Trello by Curtis, I looked into the status of forgotten_password. Going to the URL leaves a broken form (no logo, etc) as well as an internal server error.
Likely, this functionality has been left out of testing for a while and other changes have left it broken.
- Would writing a test for this be possible?
- Is there an existing scenario?
- should this be added to help.o.org? Perhaps prominently?
- How does a user find this URL without knowing about it? Link from the login page?

All of 2403 won't be applicable for 5.0.x. Could you rebase just the fix for this to dev_5_0?

This issue had been resolved in https://github.com/openmicroscopy/openmicroscopy/pull/2403 but it was put on hold. Not sure what the next step is at this moment.
