rleigh, can you check the status of the file on disk under ManagedRepo?? I.e. is this an upload issue (the sha1 is not correct) or a metadata/rendering issue?

md5sums are identical to the original files (/OMERO/ManagedRepository/user-3/2012/10/2/avi/*).
