DOC: webstart extension/modification guide

To make webstart really useful for sites, several things should be explained:
- how to modify the index.html template manually
- how to change the keystore/certs used
- how to add custom code to the webstart jars
- how to update the bioformats jars for importing
- (for later) how to have multiple webstart directories
- how to use configuration options...
- to change the heap size
- to change the name of the desktop shortcut ("OMERO.insight at my university")
- to change the icon used
